What Is Public Health?
Public health is a multidisciplinary field focused on preventing disease, prolonging life, and promoting health across entire communities and populations. Its defining characteristic is the population perspective — instead of asking "why is this person sick?", public health asks "why do some populations have higher rates of disease than others?" and "what can we do to prevent it at scale?"
The three core functions of public health are:
- Assessment: Systematically collecting and analyzing data on health status, risks, and outcomes
- Policy development: Using evidence to develop policies and plans that support individual and community health
- Assurance: Ensuring that services necessary to achieve agreed-upon health goals are provided

Incidence
Incidence is the rate of new cases of a disease arising in a defined population over a specified time period:
Incidence rate = (New cases / Population at risk) × 1,000 (or 100,000)
Incidence measures the risk of developing a disease and is the primary metric for evaluating the effectiveness of disease prevention interventions.
Prevalence
Prevalence is the proportion of a population that has a disease at a given point in time (point prevalence) or over a defined period (period prevalence):
Prevalence = Cases / Total population
Prevalence reflects the burden of disease on a population and informs healthcare resource allocation.
Mortality Rates
- Crude death rate: Total deaths per 1,000 population per year
- Cause-specific mortality rate: Deaths from a specific cause per 100,000 population
- Case fatality rate (CFR): Proportion of confirmed cases that result in death — a measure of disease severity
Epidemiology: The Core Science of Public Health
Epidemiology is the study of the distribution and determinants of health and disease in populations. It identifies risk factors, traces the origin of outbreaks, evaluates interventions, and informs clinical practice through population-level evidence. Key study designs include:
- Cohort studies: Follow exposed and unexposed groups forward in time to compare disease incidence
- Case-control studies: Compare past exposures in people with and without a disease
- Randomized controlled trials (RCTs): The gold standard for evaluating interventions — randomly assign participants to treatment or control groups
- Cross-sectional studies: Measure disease and exposure simultaneously in a population snapshot
Disease Surveillance and Control
Public health systems rely on disease surveillance — the ongoing collection, analysis, and interpretation of health data — to detect outbreaks early and monitor trends. Notifiable disease reporting, syndromic surveillance, and genomic sequencing of pathogens are core tools. Control measures include vaccination programs, contact tracing, quarantine, vector control, and health communication campaigns.

Incidence counts new cases of a disease arising in a population over a specific time period and reflects the risk of developing the disease. Prevalence counts all existing cases (new and old) at a point in time and reflects the overall burden of disease. For chronic diseases, prevalence is much higher than incidence; for acute short-duration illnesses, they are more similar.
Medicine focuses on diagnosing and treating disease in individual patients. Public health focuses on preventing disease and promoting health at the population level — through surveillance, policy, environmental interventions, and community-level programs. A physician treats a patient with tuberculosis; public health tracks transmission, traces contacts, and designs vaccination and screening programs to prevent further spread.
The case fatality rate is the proportion of confirmed cases of a disease that result in death: CFR = (Deaths / Confirmed cases) × 100%. It measures disease severity. CFR can be misleading if testing is incomplete — if only severe cases are confirmed, CFR appears higher than the true infection fatality rate, which includes all infections (including mild and asymptomatic).
Herd immunity occurs when enough of a population is immune (through vaccination or prior infection) to interrupt disease transmission, protecting those who cannot be vaccinated. The herd immunity threshold is: HIT = 1 − (1/R₀), where R₀ is the basic reproduction number. For measles (R₀ ≈ 15), HIT ≈ 93%; for COVID-19 original strain (R₀ ≈ 2.5), HIT ≈ 60%.
