Crude Death Rate
CDR = (Total deaths in a period / Mid-period population) × 1000
Expressed as deaths per 1,000 person-years. The crude rate does not account for age structure — populations with older age distributions will have higher CDRs even if they are healthier at each age. This is why direct or indirect age standardization is used for comparisons between populations.
Age-Standardized Mortality Rate
Age-standardization applies the age-specific death rates of the study population to a reference (standard) population age distribution, producing a rate that would be expected if both populations had the same age structure. The World Standard Population (WHO) or US Standard Population (CDC) are common reference populations for international and national comparisons respectively.
Case Fatality Rate (CFR)
CFR = (Deaths from a disease / Confirmed cases of the disease) × 100%
CFR measures the proportion of known cases that die. It is heavily influenced by case ascertainment — if mild cases go undiagnosed, the denominator is underestimated and CFR is artificially high. During COVID-19, early CFR estimates varied widely due to testing capacity differences across countries.
Infant Mortality Rate (IMR)
IMR = (Infant deaths under 1 year / Live births) × 1000
IMR is highly sensitive to access to healthcare, sanitation, nutrition, and vaccine coverage. It ranges from <2 per 1,000 live births in high-income countries to >50 in the least-developed nations, making it a powerful summary indicator of societal health and development.

Crude death rate (CDR) is total deaths per 1,000 population in a given period — it does not control for age structure. An aging population will naturally have a higher CDR even if it is healthier at each age. Age-standardized mortality rate applies age-specific death rates to a standard reference population, removing the confounding effect of age distribution and allowing valid comparisons between countries and over time.
Case fatality rate (CFR) = (deaths from a disease / confirmed cases) × 100%. It measures the lethality of a disease among those known to be infected. CFR is highly sensitive to testing and case ascertainment — during an outbreak, if only severe cases are tested, CFR appears high. As testing expands to include milder cases, the denominator grows and CFR falls. High-income countries typically have IMR below 5 per 1,000. IMR above 50 per 1,000 indicates severe healthcare system deficiencies. Neonatal mortality (first 28 days) accounts for about 75% of all infant deaths globally.
Mortality rate measures deaths per unit of population per time period — it quantifies how frequently people die. Morbidity rate measures the frequency of disease or illness in a population — it quantifies how often people become sick or disabled. Both are expressed per 1,000 or 100,000 population per year. A disease can have high morbidity (many people get sick) but low mortality (few die), like influenza; or low morbidity but high mortality, like rabies.
