Body Mass Index (BMI)
BMI = weight (kg) / height² (m)
WHO classification: < 18.5 = underweight; 18.5–24.9 = normal; 25–29.9 = overweight; ≥ 30 = obese. BMI has limitations: it does not differentiate muscle from fat, underestimates adiposity in older adults, and may misclassify athletes and some ethnic groups.
Basal Metabolic Rate (BMR)
Mifflin-St Jeor equation (most accurate): Men: BMR = 10W + 6.25H − 5A + 5; Women: BMR = 10W + 6.25H − 5A − 161 (W = kg, H = cm, A = years). Total daily energy = BMR × activity factor (1.2–1.9).

BMI = weight(kg) / height(m)². Normal range: 18.5–24.9. While BMI is a useful population screening tool, it has significant limitations: it cannot distinguish muscle from fat (athletes may be classified as overweight); it underestimates adiposity in older adults who lose muscle; it may be biased across ethnic groups (some Asian populations have higher cardiovascular risk at lower BMI thresholds). Waist circumference (> 40 inches in men, > 35 inches in women = increased risk) and body fat percentage are more accurate individual adiposity measures.
Per AHA 2017 guidelines: Normal = systolic < 120 AND diastolic < 80 mmHg. Elevated = 120–129/< 80. Stage 1 hypertension = 130–139 systolic OR 80–89 diastolic. Stage 2 hypertension = ≥ 140 OR ≥ 90. Hypertensive crisis = > 180/> 120 — requires immediate medical attention. Blood pressure should be measured at rest after 5 minutes, ideally at the same time of day. Two or more readings at separate visits confirm a hypertension diagnosis.
Step 1: Calculate BMR using Mifflin-St Jeor. For a 35-year-old woman, 65 kg, 165 cm: BMR = 10×65 + 6.25×165 − 5×35 − 161 = 650 + 1031 − 175 − 161 = 1,345 kcal/day. Step 2: Multiply by activity factor: sedentary (desk job) × 1.2 = 1,614 kcal/day; moderately active (exercise 3–5 days/week) × 1.55 = 2,085 kcal/day. For weight loss: subtract 300–500 kcal from TDEE. For weight gain: add 200–300 kcal.
Ideal body weight (IBW) estimates the optimal weight for a given height, independent of actual weight. Devine formula: Men = 50 + 2.3 per inch over 5 ft; Women = 45.5 + 2.3 per inch over 5 ft (in kg). IBW is used in clinical pharmacology for dosing drugs whose distribution volume correlates better with lean body mass than total body weight — particularly aminoglycosides, vancomycin, and some chemotherapy agents in obese patients. Adjusted body weight (ABW = IBW + 0.4×(actual−IBW)) is used when actual weight significantly exceeds IBW.
