CDR Formula
CDR = (D / P) × 1,000
D = total deaths in the period; P = mid-year population; units = deaths per 1,000 population per year.
Example: country with 85,000 deaths and 12 million population: CDR = (85,000/12,000,000) × 1,000 = 7.08 per 1,000.
Global CDR Values
- Global average: ~7–8 per 1,000
- High-income countries: 9–12 per 1,000 (older populations)
- Low-income countries: 8–14 per 1,000 (higher age-specific rates)
- Japan (oldest population): ~12–13 per 1,000
- Niger/Mali: ~8–10 per 1,000 (younger population despite high mortality)
Note: some low-income countries have lower CDR than high-income countries because their very young age structure (most people are under 30) — young people rarely die, offsetting high age-specific rates in older age groups.
CDR vs. Age-Standardized Rate
CDR is affected by age structure; age-standardized mortality rate (ASMR) removes this effect by applying a standard age distribution (WHO World Standard Population). ASMR enables valid mortality comparisons between populations. Use CDR for: historical trend analysis (stable population); preliminary demographic assessment; population projections with other components.

CDR = (Deaths / Population) × 1,000. It gives deaths per 1,000 population per year. Example: 46,000 deaths in a country of 5.8 million people: CDR = (46,000/5,800,000) × 1,000 = 7.93 per 1,000. Use the mid-year population estimate for the denominator (average of population at start and end of year). CDR is 'crude' because it does not account for age composition — a very important caveat when comparing populations.
CDR is heavily influenced by age structure. An elderly population has a higher CDR than a young population even if individuals at each age are equally healthy. Example: Japan (median age ~49 years) CDR ≈ 12/1000; Niger (median age ~15 years) CDR ≈ 9/1000 — even though Niger has far higher mortality at every age, its very young population keeps the crude rate lower. This 'ecological fallacy' makes direct CDR comparisons misleading. Solution: use age-standardized mortality rates (ASMR) which apply actual death rates to a standard age distribution, removing the age structure effect. WHO and Global Burden of Disease use ASMR for cross-country and trend comparisons.
Natural rate of increase = CBR (crude birth rate) − CDR. If CBR = 25 per 1,000 and CDR = 7 per 1,000: natural increase = 18 per 1,000 = 1.8% natural growth per year (not accounting for migration). When CBR = CDR: zero natural growth (population neither grows nor shrinks). Stage of demographic transition determines CBR-CDR relationship: Stage 2 (developing countries): high CBR + falling CDR → rapid growth; Stage 4 (developed countries): low CBR + low CDR → near-zero growth. Demographic transition theory predicts CDR falls as living standards improve, with CBR following later.
CDR: total deaths from all causes per 1,000 population. Cause-specific death rate: deaths from a single cause per 100,000 population per year. Example: cardiovascular disease death rate = (CVD deaths/population) × 100,000. Note: cause-specific rates are often expressed per 100,000 rather than per 1,000 because deaths from specific causes are relatively rare. Age-standardized cause-specific rates allow comparison between populations. Proportional mortality: what fraction of all deaths is caused by a specific disease — this is NOT a rate (denominator is deaths, not population) but is useful for describing disease burden within a population.
